1L22 15:55 King's Cross - Harrogate, to Peterborough (15 minute delay Peterborough).
Casualty report states: boiler failure - burner motor fuse blown.
Conclusions: 26.01.70 DME KX states: Perhaps the DME Leeds will take up with his footplate staff
to ascertain whether the spare fuse was tried before they requested a fresh locomotive
.
Following letter dated 13.03.70 DME Leeds states: With reference to the concluded casualty report
dated 26.01.70, by the DME KX, there is no Driver Swale in the Leeds division - on 23.01.70 the 1L22
was worked by Driver North with Secondman Cooke, who was questioned and has no knowledge of
the delay or incident in question
.
41,000 miles since last classified repair (18.10.69)

.

King's Cross Loco.
1S77 23:20 King's Cross - Edinburgh, to Peterborough (3 min delay Stevenage & 19 at Peterborough - 9016 fwd 24/01).
Casualty report states: no exhauster sped up No.1 end - exhauster speed up switch out of adjustment
due to check nut loose
.
Driver F Hughes (King's Cross) reports: The vacuum and air brakes on 9020 were tested by myself
before leaving Loco yard and they were normal. But after being coupled to the train vacuum brake
would not rise above 15 inches, I asked the C&W examiner to look at the train and also asked for
our own fitter to examine the loco brake again
. The exhauster exciters would not make any difference
to the vacuum and it took 3 to 4 minutes before I got to 20 inches
. The loco fitter said it was the train
brake and that the locomotive was okay to go
. I had to make a brake application approaching
Stevenage and Holme and was brought to a stand both times
. Control were informed of trouble at
Stevenage and I requested another locomotive from Peterborough
. This defect had been booked two
days before and the fitter remarked 'tested and found okay on shed', the exhauster exciter was useless
when coupled to a train
.
Conclusions: DME KX states: This switch had received attention ten days earlier when the locomotive
was a casualty at Newcastle - it would therefore seem that the check nut had not been tightened
correctly after this first incident and no doubt the DME Newcastle will investigate. However, in view
of these two casualties I have instructed my Depot Manager at Finsbury Park to specially check this
switch at the next 'B' examination
.

12 - 1S16 08:00 King's Cross - Edinburgh (6 minute delay between King's Cross & Potters Bar).
Locomotive accompanied in traffic by Technical Inspector
.
Casualty report states: Momentary brightening of wheel slip light
.
Driver C Ward reports: After leaving King's Cross and at a speed of 30 mph, with controller
wide open, there occurred a wheel slip indication and sudden loss of amps (amp needle to zero)
.
After closing the controller and then opening again fully amp needle came round to just over
1500 and then overloaded again with momentary brightening of the wheel slip light
. Inspector
A Richardson was riding on this engine at the time, as the repair book showed an earth fault
had been booked a few days previously, it was decided to press the earth fault button and with
just No.2 engine running we had no trouble, except loss of time to Potters Bar, though only
reaching 45 mph
. After getting over Potters Bar No.1 engine was restarted and we had no
further trouble
.
Haymarket TMD.
1E25 16:00 Edinburgh - King's Cross, to Newcastle (10 minute delay Newcastle - 164 forward).
Casualty report states: wheel slip at 60 mph
.
Conclusions: 17.06.70 DME KX states: Neither Gateshead nor Finsbury Park Depot could fault
this locomotive but in view of the fact that there had been a previous similar report earlier in the
same day I am inclined to think that there must have been an intermittent defect present
. The
locomotive was accompanied in service by the CM&EE's Chief Inspector Mr Edwards, but the
fault could not be reproduced and there was no further trouble up to the locomotive entering
Main Works on 14.05.70

reports of intermittent false wheel slip indications on this locomotive
. It has been accompanied
in traffic by Technical Staff several times but the fault has not reproduced itself on these occasions
.
The CM&EE's Chief Inspector Mr Edwards has also checked the current flowing through the

train until 17:10
.
20,123 miles since last classified repair (06.06.70)
.
Conclusions: 27.07.70 DME KX states: Trouble has been experienced on a number of locomotives due
to this valve sticking and it may be that special attention will have to be given to this valve during
examinations, but before recommending this action, a check should be made to establish if similar
trouble has been experienced at other depots

Casualty report states: boiler failure - water pump not working
.
27,443 miles since last classified repair (06.06.70)
.
Conclusions: 11.09.70 DME KX states: With reference to the above casualty report, I am now advised
by the DME Newcastle that a driver's report has revealed that the boiler water tank was empty on
arrival at Newcastle
. The tank was refilled but upon attempting to restart the boiler the water pump
would not function correctly
. The locomotive was taken off the train but before it reached Gateshead
Depot the pump had picked up water and the boiler was functioning correctly
. It would appear therefore
that this was a case of a temporary air lock caused by the tank having previously emptied below the
level of the suction pipe
.

Casualty report dated 07.12.70 states loco off 'C' exam
.
1S79 23:35 King's Cross - Edinburgh, to Newcastle (delayed Relly Mill - 9008 forward 05/12).

Casualty report states: No.1 engine shut down - overspeed tripped several times
.
141,000 miles since last classified repair (06.06.70)
.
Conclusions: 30.12.70 DME KX states: This locomotive was subsequently checked in service by
the CM&EE's Chief Inspector Mr Edwards who suspected that No.1 engine governor was defective,
and this was changed at Finsbury Park Depot on 10.12.70. The number of Deltic engine governors
requiring renewal at Depot has increased considerably in recent months (six governors have been
changed at Finsbury Park since 01.01.70) and I understand from minutes of the Type 5 Maintenance
Meeting that this is a matter which is being investigated
.
5 - Gateshead TMD.
Repairs - Overspeed & engine rpm checked, no faults found
. The coolant level was also down on
No.1 engine due to several leaks from hoses and 8 gallons of coolant were supplied and hoses
nipped up
. Regional control advised further check at King's Cross
.

Casualty report states: boiler defective - safety valve blowing through continuously
141,000 miles since last classified repair (06.06.70)
.
Conclusions: 30.12.70 DME KX states: It would appear that the safety valve lifted whilst the
locomotive was being backed on to its train and then stuck open
.

Casualty report states: fire in engine room on 'C' bank side of No.1 engine - No.1 main generator
flashed over and cable ducting & cables burnt 'C' side No.1 end
.
149,000 miles since last classified repair (06.06.70)
.
Conclusions: 30.12.70 DME KX states: It would appear that the fire damage was initiated by a
flashover on No.1 main generator
. This particular locomotive had suffered two previous flashovers
in the preceding two months without apparent cause. Flashovers continue to be one of the biggest
unresolved problems on this class of locomotive
.
To Doncaster Works - Light repair.
Flashover damage on No.1 main generator and burnt wiring in trunking - power units No.1 406 replaced by
447 & No.2 435 replaced by 426, bogies No.1 9000-47 replaced by 9000-3 & No.2 9000-22 replaced by
9000-4 and boiler 5090/J3241 was given an Unclassified repair. The body was given a Light repair and the
locomotive was fitted with ETH equipment
