| 22 - | 1E01 08:00 Edinburgh - King's Cross. King's Cross Loco. 1S42 16:00 King's Cross - Edinburgh, to Peterborough (16 minute delay Peterborough - 274 forward). Casualty report states: Wheelslip indication at speed. Driver C Howard (King's Cross) reports: Wheelslip relay operating - persistent fault started after Biggleswade, diesel was losing time so I requested a fresh engine at Peterborough. 5,700 miles since last classified repair 913.02.71) - 15,700 stated. Conclusions: 31.03.71 DME KX states: No apparent fault at the time but subsequent checking in traffic revealed that the traction motor circuits were out of balance - the loco was taken out of traffic for Main Works rectification on 16.03.71. Please see correspondence regarding false wheelslip indication on locomotives No.s 9015 & 9018 after release from Works. I would suggest that a voltage check across the wheelslip relays when the locomotive undergoes its load test run after a Classified Repair would be desirable. Peterborough TMD. Repairs - no fault found. |

Casualty report states: warning device defective No.2 end & Secondman's side control valve blowing through - rubber seating dislodged and twisted. 19,250 miles since last classified repair (13.02.71). Conclusions: 05.05.71 DME KX states: There have been three cases recently of defective horn valves on class 55 locomotives. See also casualties for 9003 (18.02.71) and 9001 (21.04.71). Perhaps some investigation into the Works repair and or overhaul procedure would be desirable, with particular reference to the rubber seating washers (complete valve subsequently changed at Finsbury Park). Doncaster TMD. Repairs - control valve rubber seating replaced correctly and tested. |

Doncaster (19 minute delay Doncaster - 9005
forward). Casualty report states: false wheelslip indication at 80mph and above. 32,800 miles since last classified repair (13.02.71). Conclusions: 26.05.71 DME KX states: Please see my letter of 19th March and the CM&EE's reply of 26th April regarding false wheelslip indications experienced with this locomotive and results of special shopping. Resetting of the wheelslip relay has now apparently overcome this trouble, but it is obvious that the out of balance voltage at high speeds is greater than is desirable. In view of the amount of trouble experienced recently with false wheelslip indications on this class of locomotive I consider that a check of out of balance voltages, at 100mph with warm machines and the pick up voltage of wheelslip relays, should be made before a locomotive is released to traffic after Classified Repair. Doncaster TMD. Repairs - could not fault, locomotive to be accompanied in traffic by CM&EE Inspector on 1A29. |
| 28 - | Doncaster TMD. 1A29 16:30 Doncaster - King's Cross. Mr Edwards CM&EE on footplate traced fault to WSR2 picking up at 28 volts. Finsbury Park TMD. Repairs - WSR2 reset to 32 volts. |

Casualty report states: warning horn not working (isolated en-route due to sounding continuously). 37,750 miles since last classified repair (13.02.71). Conclusions: 04.06.71 DME KX states: This is yet another case of a defective horn valve on this class of locomotive due to an 'O' ring becoming displaced. York TMD. Casualty report states: engine cutting out - intermittent power earth fault on No.2 end cubicle bus bar from reverser to 4, 5 & 6 traction motor contactor fouling framing of the cubicle (due to the vertical length of the bus bar being bent). 44,060 miles since last classified repair (13.02.71). Conclusions: 22.07.71 DME KX states: I am confident that the bus bar could not have been bent accidentally whilst in situ as its cross section is too great. I can only assume therefore, that it had not been set correctly by Main Works, although I appreciate that the locomotive had been in service over three months before the defect showed itself. Gateshead TMD. Repairs - bus bar straightened and reinsulated. |

Casualty report states: loss of power - load regulator finger contacts burnt. 95,920 miles since last classified repair (13.02.71). Conclusions: 26.08.71 DME KX states: The history of this locomotive shows several instances of defects in No.1 main generator field circuit, but no root cause has been established. On 24th August the GFG1 contactor was found to be badly burnt and on 17th September the locomotive was checked in service by the CM&EE representative Mr Edwards who reported an open circuit on R33 stud. Mr Edwards along with my staff, however, subsequently checked the locomotive in Depot but could not fault it. |

Casualty report states: Driver reported compressors stopped, air lost and brakes applied. 130,980 miles since last classified repair (13.02.71). Conclusions: 24.01.72 DME KX states: I can only assume that when the locomotive failed No.1 engine had in fact shut down and due to some intermittent defect in the electrical control circuits 'AGC3' did not close to allow No.1 compressor (vacuum braked train) to run from No.2 auxiliary generator. If the driver had either used the fuel priming pump button for No.1 engine and attempted to restart that, or the compressor emergency switch he would in all probability have been able to regain air pressure. There has been no further trouble with this locomotive with regard to either compressors or fuel system air locking. |
| 20 - | York TMD. Repairs - unable to fault compressors or compressor circuit but unable to start No.1 engine due to fuel system air locked (No.1 engine not running when locomotive arrived on York Depot). No.1 engine fuel system primed, engine restarted, run up and tested. |

Casualty report states: short of fuel - train stabled at Berwick and loco light engine to Gateshead. 136,170 miles since last classified repair (13.02.71). Conclusions: 30.12.71 DME KX states: locomotive log book checked and no record of servicing at Haymarket - appears to be mismanagement in Scottish Region. 0N20 Berwick - Gateshead. Gateshead TMD. Arrive 07:45 - 685 gallons supplied. No.1 engine shut down due to fuel aeration - fuel system bled and engine tested. 1A15 09:05 Newcastle - King's Cross, "The Tees-Tyne Pullman" (32 minute delay Newcastle awaiting loco). Casualty report states: No.1 engine shutting down - fuel starvation due to fuel aeration. Conclusions: 03.12.71 DME KX states: locomotive No.1516 was diagrammed for this train, but due to a series of mishaps 1516 was taken to work 1A11 08:10 Newcastle - King's Cross. 9015 had been fuelled with 700 gallons after arriving on the depot at 07:45 and it was after the fuelling that the above trouble developed. |
